We are so thrilled that you are ready to enroll your child at Creative Care Christian Preschool. You have completed the first step towards enrollment!

In order to complete your child's registration, you must submit additional forms linked below:

Step 2: Registration Forms

Step 3: Medical Report (to be completed by your child's doctor)

Please note, your child's spot in any desired program is not secured until all forms have been properly submitted and $300 enrollment fee has been paid.
